I'm a vegetarian who happens to be mildly lactose-intolerant, so it makes sense to keep a list of things that (perhaps unexpectedly) turn out to be vegetarian, vegan, dairy-free, etc. This way, I don't have to keep redoing my research. =)

- plainer varieties are usually ok. anything with a flavouring needs to be scrutinized for milk solids

Tim Hortons
Tim's soups without obvious meat are vegetarian. The exception is minestrone (pork & beef flavouring). The hearty vegetable soup is vegan.
Vegetarians can eat many things off the menu but slim pickings for vegans here: dry bagels (possibly improved by tomato & lettuce), oatcakes, bread rolls, black coffee & tea.
- their liquid margarine (the stuff that goes on their english muffins) appears vegan.
hash browns and fries are now vegan & cooked in vegan oil but the fries contain trans fats.
- the hotcakes contain whey and are thus only vegetarian.
apple slices: the caramel dip contains milk & butter.
- pies contain l-cysteine which can be made from damn near anything.
Pizza Hut
they list all their ingredients for everything they make on their web site -- even down to the dinner mints! -- which is incredibly helpful.
Dough: pan and thin & crispy are vegan. Classic crust contains dairy.
- Sauce: "sweet pizza sauce" contains romano cheese. Pan and "4 for all" sauce is vegan.
- This is yet another fast-food place that sports a rather artificial vegan poutine sauce.
Wendys
- slim vegetarian pickings here: baked potatoes, frosties, drinks. even the salads all have meat.
Culvers
They put meat and dairy into damn near everything. Their own web site says that "None of Culver's menu items are certified as vegetarian; all menu items may contain trace amounts of ingredients derived from animals."
Au Bon Pain, Panera, Noodles and Co, Chipotle, Moe's Southwest Grill
- Great options when state-side.
